I, the undersigned participant, hereby acknowledge and agree to the following terms and conditions in consideration for being allowed to participate in the Brazilian Jiu-Jitsu seminar organized by Carlos Machado Jiu-Jitsu Association (hereinafter referred to as "Organizer").
Assumption of Risk:
I am aware that participating in the Brazilian Jiu-Jitsu seminar involves inherent risks, including but not limited to physical injury, illness, or death. I voluntarily assume all such risks and acknowledge that I am responsible for my own safety.
Medical Condition:
I hereby certify that I am physically fit to participate in the seminar and have not been advised otherwise by a qualified medical professional. I will notify the Organizer of any medical conditions or concerns that may affect my ability to participate safely.
Release of Liability:
In consideration for being permitted to participate in the seminar, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless the Organizer, its officers, agents, employees, and volunteers from any and all liability, claims, demands, actions, or causes of action, whether in law or equity, arising out of my participation in the seminar.
Emergency Medical Treatment:
I understand that in the event of an injury or illness, the Organizer may need to seek emergency medical treatment on my behalf. I authorize the Organizer to seek such treatment and agree to be responsible for any associated medical expenses.
Use of Likeness:
I grant the Organizer the right to use photographs, video recordings, or any other media taken during the seminar for promotional purposes without compensation.
Contact Information:
I provide my name, phone number, and email address to the Organizer for communication purposes related to the seminar. I understand that this information will be kept confidential and used solely for seminar-related matters.
